A 55 yr old male patient came to causality with chief complaints of cough since 10 days 
Associated with left sided chain pain since 10 days

HOPI:Patient was apparently asymptomatic since 15 days and then developed FEVER since 10 days  fever was intermittent,low grade,not assosiated with chills and rigor

assosiated with COUGH since 10 days cough is productive withwhite color,non-foul smelling sputum

Cought is associated with LEFT SIDED CHEST PAIN since 10DAYS
Pain is non radiating,
Not assosiated with sweating

Associated SHORTNESS OF BREATH
since 10 days
NEGATIVE HISTORY:
No history burning micturition
                   Loose stools
PAST HISTORY:
History of hypertension since 15 days
No history of surgeries,trauma.
FAMILY HISTORY:
no significant family history
PERSONAL HISTORY:
Diet-mixed
Appetite-reduced
Bladder-normal
Bowel-normal
Addictions
Alcohol-occasionally
Cigarette -2-3/day since 30yrs
GENERAL EXAMINATION:
Patient was conscious coherent and well oriented with time place and person..

NO pallor,icterus,cyanosis,clubbing,koilonychosis,lymphadenopathy,edema
VITALS:
BP-120/80
PULSE-70beats/min
TEMP
RESPIRATORY RATE....
